Responsibilities:
General Description:
Essential Functions:
Perform recruitment, intake/eligibility, counseling, verification, approval, assessment, placement, and referral services to prospective clients and children.
Perform post placement supervision, re-verification, and additional/ appropriate duties necessary in administering designated services.
Assist with the Continuous Quality Improvement Program (CQI) for Family Services.
Plan and conduct necessary meetings, seminars, interviews, and home visits.
Assume role of liaison worker with local agencies, community resources, and other geographical agency offices.
Work to develop community resources including recruitment and training of families and volunteers.
Prepare, compile, submit, and maintain accurate records, files, forms, statistics, and additional relevant information in accord with Agency policy, licensing and/or funding requirements.
Review and sign off on client records in accordance with licensing, CARF, Youth for Tomorrow Foundation (YFT), and contract standards.
Participate in planning, coordinating, implementing, and evaluating designated social service program(s).
Extensive driving required.
Some travel required.
